Fix Claude Code Losing Context (2026)
Thirty minutes into a refactoring session, Claude Code forgets the naming convention you agreed on ten messages ago. It starts duplicating functions it already wrote or contradicts decisions from earlier in the conversation.
The Problem

Every Claude Code session consumes tokens. Messages, file contents, tool outputs — they all accumulate. When the context window fills, the system compacts older content. Critical decisions from message 5 may be gone by message 50.
This is a fundamental constraint of transformer architectures, not a bug. But it can be managed.
The Fix
Use the task decomposition pattern from claude-task-master (27K+ stars) to break long sessions into discrete, completable chunks. Task Master parses project requirements into structured task lists, each small enough to fit in a single session.

This generates a tasks/ directory with numbered, dependency-ordered tasks that Claude Code can execute one at a time.
Step 1: Create Session Checkpoints
Add a checkpoint section to your CLAUDE.md:

2. Include: what was done, what files changed, what comes next
3. Start each new session by reading PROGRESS.md first
Step 2: Chunk Your Work

Session 2: "Continue from PROGRESS.md — add refresh token support"
Session 3: "Continue from PROGRESS.md — write tests for auth changes"

- After 15 messages, ask: “What decisions have we made so far?”
- Claude Code should reference PROGRESS.md and give an accurate summary
- If it misses decisions, your checkpoint frequency needs to increase
